The Ohio Department of Public Safety (ODPS) has completed an updated version of the OH-1 form, Ohio's statewide uniform crash report form. Beginning January 1, 2012, crash reporting agencies may begin submitting paper and electronic reports using the new form. During 2012, agencies may continue to submit crash reports using the old form in order to utilize existing supplies; however, they are encouraged to transition to the new form as soon as possible. As of January 1, 2013, only the new OH-1 form will be accepted by ODPS; crashes submitted using the old form will be returned to agencies for correction and resubmission.
